Our physicians have all earned MD degrees from accredited US medical schools and completed years of additional dermatology residency training to become Board Certified experts dedicated to this medical specialty. Meet our Physicians and Physician Assistant


Carl Bigler M.D.

CARL BIGLER, M.D. has been practicing dermatology in Northern Arizona since 1995. He is board certified in Dermatology, Dermatopathology, and Internal Medicine, and has special expertise and experience in skin cancer surgery and facial reconstruction. Dr. Carl Bigler grew up in Western Kansas, completed his undergraduate education at Williams College in Massachusetts, medical school at Washington University in Saint Louis, and residencies at the University of New Mexico (medicine) and University of Colorado (dermatology and dermatopathology). Prior to joining the Northern Arizona Dermatology Center, he was a full time faculty member at the University of New Mexico Medical School. He enjoys his patients and the full spectrum of his practice. Carl lives in Flagstaff with his wife Jane, and four children. Outside of the clinic, he may be found biking, hiking, skiing, reading, or involved with his children's numerous activities.

Carin Cain M.D.

CARIN CAIN, M.D. is a graduate of Mayo School of Medicine. She received her Dermatology training at the University of Iowa Hospitals and Clinics, after which she served as a Chief of Dermatology in the United States Air Force both in the U.S. and overseas. As part of the physician staff at Northern Arizona Dermatology Center since 1998, Dr. Cain is gifted in both general and surgical dermatology, including Mohs Micrographic Surgery. Dr. Cain is a fellow of the American Academy of Dermatology, and maintains memberships in the American Society of Dermatologic Surgeons and the American Medical Association. Born in Greensboro, North Carolina, she grew up in Southern California. She now resides near the red rocks of Sedona, where she enjoys hiking and skiing, as well as cooking and remodeling houses.
Kelly Czarnecki PA-C

KELLY CZARNECKI, PA-C graduated from Truman State University in Kirksville, Missouri with a B.S. and a B.A. in Biology. Ms. Czarnecki then attended the Arizona School of Health Sciences, graduating with a Master's degree in Physician Assistant studies. After receiving her Physician Assistant certification, Ms. Czarnecki worked in the field of Rheumatology for two years, then completed a postgraduate fellowship in Dermatology. She has worked in Dermatology for seven years. Ms. Czarnecki enjoys the field of Dermatology because of the variety of patients, and surgical procedures. Kelly Czarnecki and her family moved to the wonderful community of Flagstaff in 2005, and has enjoyed mountain biking, kayaking and the beautiful surroundings. She also enjoys cooking and yoga.

Scott Dale M.D.

SCOTT DALE, M.D. joined this dermatology practice in 2002. He completed his undergraduate studies in the Liberal Arts at Carleton College in Minnesota. Scott switched from an early career in business to study the biological sciences at University of California San Diego. A couple of years later, he attended medical school at the University of Chicago Pritzker School of Medicine. Dr. Dale interned in General Medicine, and completed Dermatology residency at The University of Chicago Hospitals. Dr. Dale has been practicing medical and surgical dermatology, including Mohs Micrographic Surgery at Northern Arizona Dermatology Center since graduating from the University of Chicago. Dr. Dale is Board Certified in Dermatology, a member of the American Society for Mohs Surgery, and a fellow in the American Academy of Dermatology. Dr. Dale's passions include playing outdoors, cooking, wine, travel, and raising his daughter.
Therese Holguin M.D.

THERESE HOLGUIN, M.D. is a graduate of the University of New Mexico School of Medicine. After two years of Internal Medicine residency at the University of Kansas Medical Center, Dr. Holguin returned to the University of New Mexico, where she completed her Dermatology residency, and remained two years longer as Assistant Professor. Northern Arizona Dermatology Center has been the location of her professional practice since 1990. Dr. Holguin's practice consists of general dermatology with an emphasis on the diagnosis and treatment of skin cancer including the Mohs technique. Dr. Holguin is certified to perform cosmetic surgery.

Edward Kauffman M.D.

EDWARD KAUFFMAN, M.D. has been practicing dermatology since 1977. After graduating from the University of Arizona in Tucson in 1968, Dr. Kauffman went on to receive his medical degree in 1972 from Washington University in Saint Louis, Missouri. Dr. Kauffman completed his formal dermatology training at the same school, and became board certified in Dermatology. Most of his medical practice has been based in Bellingham, Washington, where he was also Chief of Staff at Saint Joseph's Hospital from 1988 to 1989. Since moving back to his home state of sunny Arizona, Dr. Kauffman has become certified in Mohs surgery and enjoys practicing general dermatology. Most every weekend will find Dr. Kauffman hiking the trails near Sedona with his dogs.
Cynthia Knutson M.D.

Cynthia Knutson, M.D. Received her Medical degree from St. Louis University School of Medicine in 1988. After completing her residency in Pediatrics at the University of Arizona Medical Center, she obtained her Dermatology training at the University of Texas Southwestern in Dallas. Dr. Knutson practices general dermatology, and enjoys seeing patients of all ages. She is a Fellow of the American Academy of Dermatology, and maintains memberships in the Society for Pediatric Dermatology and the American Society of Dermatologic Surgery. Dr. Knutson has been with Northern Arizona Dermatology Center since 1996. Doctor Knutson and her husband John enjoy everything about living in Flagstaff, and feel very fortunate to raise their kids here.

Jordan Miller M.D.

JORDAN MILLER, M.D. is a graduate of the Medical College of Wisconsin in Milwaukee where he also completed his dermatology residency. A native of Northern Arizona, Dr. Miller attended Brigham Young University and graduated in 2000. Dr. Miller joined this practice in 2009. He is Board Certified in Dermatology and his practice centers on medical and surgical dermatology, including Mohs' Micrographic Surgery. Dr. Miller, his wife and three children are enjoying their new life in Flagstaff. Dr. Miller also enjoys reading, sports and woodworking.

NAZ dermatology staff memberOur services include general, cosmetic, and surgical dermatology, including highly specialized Mohs micrographic skin cancer surgery.

All seven of our dermatologists are nationally Certified by the American Society of Mohs Surgery (www.mohssurgery.org). They surgically excise dangerous head and neck cancers daily using the Mohs Micrographic Surgery technique which is the most advanced and effective treatment procedure for skin cancer available today. Because each physician is specially trained in surgery, pathology, and reconstruction, Mohs surgery has the highest success rate of all treatments for skin cancer - up to 99 percent. The Mohs technique is also the treatment of choice for cancers of the face and other sensitive areas as it relies on the accuracy of a microscopic surgical procedure to trace the edges of the cancer and ensure complete removal of all tumors down to the roots during the initial surgery.

Our dermatologists' eyes are uniquely trained to analyze the skin, hair, and nails. They can spot potential problems and diagnose a multitude of conditions that range from minor to life-threatening.

These skills and knowledge can improve the lives of patients of all ages — the infant with a red birthmark, the child with eczema, individuals with sun-damaged skin, the senior citizen with shingles — effectively treating their skin conditions and making them look and feel their best.

Our doctors are also a great resource for learning how to prevent skin, hair, and nail problems. They can teach you how to protect yourself from the sun to avoid skin cancer, educate you about types of hair loss, and advise you about how to care for your nails to prevent common conditions such as nail fungus, for example.

Our cosmetic services include the administration of BOTOX to minimize facial wrinkles, and the removal of various benign growths and age related changes. Our dermatologists will address your concerns and guide you to your best option. We caution you to inquire about any cosmetic practitioner's credentials. Authentic Botox, for example, is only sold by the manufacturer to licensed Medical Doctors who have an "M.D." degree. Lasers and other invasive procedures should only be used under the supervision of an "M.D." as well.

Furthermore, Northern Arizona Dermatology Center is equipped with its own histopathology lab and full time laboratory staff to ensure timely results on both routine biopsies and Mohs surgical specimens.
